Please find attached the Sick Leave Bank form that must be filled out whether you wish to participate in the Sick Leave Bank or not.
- Everyone should fill out a form.
- All forms need to be turned in to me at MHS via school mail or in person – Melanie Kassing at MHS.
- All forms are due Friday, May 13, 2016. If the sick bank form is not received by me by Friday, May 13 your status will be the same as this year. Late sick bank forms will not be accepted! Again, if your form is not received by the deadline your status remains the same as this school year. No exceptions!
If you work with any full-time employees who do not receive school e-mail, please print a copy of the form for them.
If you are returning, after having left the Sick Leave Bank, you will need to donate 3 days. Please see the contract language on the Sick Leave Bank form or please refer to the contract found at http://mea19.org
We will not need to ask for days from existing Sick Leave Bank members for the upcoming school year.
